KPI dashboard - health plans management of patients with chronic disease

The Tableau dashboard highlight’s the client’s success in helping health plan members with chronic conditions access services they need to stay well. I layered Tableau on top of a SQL database, where I integrated researcher-generated analysis on health plan quality of care.
